What to include

A report with these five things is usually resolved in one reply. Without them, the first reply is a request for them.

The URL of the affected site

Not a local site, if you can avoid it. Many PWA behaviors differ on localhost.

What you expected and what happened

Including the exact wording of any error.

Where you saw it

Browser, version, and operating system. PWA support varies enough that this often is the answer.

What the service worker panel shows

Open developer tools, go to Application > Service Workers, and say whether one is registered and activated. Include any console errors.

Your versions

Progressify version, WordPress version, PHP version, and whether Pro is active.
If the problem is visual, a screenshot of the browser’s Application > Manifest panel is worth more than a screenshot of the page. It shows what the browser actually read, rather than what you configured.

Reporting something that looks like a bug

Try to narrow it before reporting. Switching to a default theme and deactivating other plugins takes a few minutes and tells you whether the problem is Progressify or a conflict. Say which of those you tried, and what changed.

Feature requests

Send them to the same address. Say what you are trying to accomplish rather than the feature you have in mind; there is often an existing way to do it.
